How to file a grievance with Kaiser Permanente You can file a discrimination grievance with Kaiser Permanente if you believe we have failed to provide these services or unlawfully discriminated in another way. You can file a grievance by phone, by mail, in person, or online. Please refer to your Evidence of Coverage or Certificate of Insurance for details. You can call Member Services for more information on the options that apply to you, or for help filing a grievance.
